loose shifter footrest

Hey all. I noticed my left footrest was loose while riding. I took a look at it and the screw that holds it in is tight as can be but the peg still is very loose. When the peg moves, the screw does not move with it.

I took off the peg but I really do not know what I am looking for. I put the peg back on and I can see in from the other side it does not stay rested in the groove. Has anyone had this problem? Sorry I am not very mechanical, here are some pics. Not sure if part number 10 is put on correctly.

These are after I put the peg back on.





Looks like the rubber from part 10 was coming out of the back in the original before pic and now I have it coming out towards the front of the bike. I'm going back to tinkering with it but I'd thought I post here while doing so.
