1. Pre-existing coverage has more protections under the ACA. To keep it simple "coverage" means treatment. So insurance companies may be required to provide "coverage" but there is no limit for cost. So if one has asthma the insurance must cover it, but they can charge 150,000 a year to do it.
2. Lifetime caps. Insurance in the individual payer networks had lifetime coverage caps. Some as low as 100,000. So once that cost was met you were on your own after that, and they could legally continue to bill you.
3. Bringing up nonsense President's say. "
