Forward Controls

Hi Routys forward controls are simply awesome but I am now building a Cafe Racer and have removed them.
I have managed to sell them here in NZ but couldn't remember what hardware it came with, And I have missed placed the installation instructions.
1) main body x2
2) Brace rod plus 2 screws
3) Connecting rod for gear shift
4) Connecting hardware for break

Cant remember if we used our existing engine mount bolts?
Can someone please scan and attach a copy of the installation instructions please. paulmrshll@gmail.com

Surely SOMEONE has that information available.. we can talk about it and keep it up on the page till someone comes along..
