This is what I did. I extended my highway pegs out 4" with stainless 1/4"  steel bar and stainless steel bolts. Could have gone to 5 or 6", but only did 4". It's fine for me. If you have really long legs you may want to go to 6".

 These are my 6" extenders. Because the engine guards are angled I had angle the extenders outward to get them level. If I had a way to bend them that would be better, but since I only have a 31" inseam so they work fine for me.     Found a way to bend the extender, A sledge hammer and a bench vice. The front wheel looks like it is close, but there is plenty of room.

 Just modified the pegs to have a heel rest. They both fold up.  I used some Buddy Footpegs by Bikemaster. Got them for $10 from here: http://www.cruisercustomizing.com/buddy-footpegs-10mm/part/BM-171250
