Drive pulley nut lossened! GRRRRRR

So today I'm right out of the annual bike inspection, paid all my insurances, stickers, taxes and all, it's all done and I'm coasting to a stop at a traffic light... life is grand. All the sudden - a large metal chunk passes me!. I hit the brake - there comes a large washer! What the... Shift to first, it whirrrrs and doesn't move. Pull up to the side, I see the drive pulley hanging on the very end of the shaft. Oil dripping around the shaft. Fuk fuk fuk and fuk!!!!!! Luckily I found all the pieces. I just hope I didn't grind the thread.

Is it really wrong to laff at a post like that? That was a lousy moment,, How did the nut get off? The washer is supposed to be bent over to hold the nut, & the washer is supposed to have a D shaped hole, so it wont spin on the shaft.

Khm... but washer? Which washer? I got the one with star shaped hole. Goes before the pulley.... perhaps I did not pick up all parts afterall... Come to think of it... I don't think I ever had one.
I don't know how it got off, it was tight and torqued right. Worked for a year now. Well, this time I'm gonna loctite it and even think about running a hole throught the nut and axle and riveting the darn thing in place. Must change the o-ring first.

Not a D, it fit's the splines.

Mine never came off,.. but it kept coming loose, 5 or 6 times in a year...
Finally, gave up, and bought a new splined washer...
I've decided that they are basically one time use...
...any time you pull the pulley, replace that washer...

so, the "star shaped" thing is actually it?? OK, then I have it. And it was there. I'll just treadlock the thing. I actually really think of riveting it. I have 4mm of spacers under the pulley to cover for bigger tire too. Must have something to do with it.

There's your problem... I don't think the splined washer will do anything with those there...
... a rivet's not gonna' hold... but if you must have the extra washers,... I'd think about drilling and pinning the nut somehow...

Sorry,, forgot bout the splined part,, anyway, the reason its splined is so it wont spin on the shaft, so when ya bend the washer over onto one of the flats on the nut, tht locks the nut so it cant come off,, IF its not torqued enough then the pulleywill wiggle enough to wear the pulley out on the shafts splines.

well... can't remember now, but it was torqued to specs. It still broke loose. Regardless, I'll loctite the heck out of it and drill and pin is what I had in mind, but instead of a pin - I'm thinking rivet. Metal rod with squashed ends, not an industrial or pop rivet.

What you plannin on drilling? If its that output shaft, youre not gonna drill that I betchya,, Fold the washer over,, its been doing the job for 20 years.

Well... washer was (past tense) doing the job, all right...
I will
try
to drill straight through the thightened and torqued nut
and
the shaft, yes. Then run either a steel cotter pin or a rivet through the hole. If drilling is at all possible...
Loctite is another option. Or both. Dunno yet.

That shaft isnt mild steel, That thing is Hard,, I doubt seriously youll drill it. A new washer & youre in business.

Well.. found some time today and got it assembled, cant drill since there's no space for the chuck. Shaft is not hard at all. Neverless, loctite and tightened way past the torque. I marked the nut, so I'll kno if it starts moving again. BTW, I never had the splined washer.. I mean, I did, behind the pulley... I used it as an additional spacer
