The wheel does appear to be offset but it is just an illusion. as it is still in line with the front wheel that way.
Back to the squeak part....it is almost impossible to get the belt to be perfectly centered on the pulley. There is a little bearing play in the pulley and it mounts to the wheel with rubber bushings, so the pulley isn't so rigidly mounted that you can really center the belt. One small turn on the adjusters can make it go from one side to the other quickly. Besides there is only about 1/16" gap between the belt and pulley rim anyway.
The squeak is from either dirt or too much tension on the belt. Get the wheel adjusters positioned equally with the correct belt tension, and then make sure the belt is clean. That should fix you up.
