Are there any aftermarket gear shifters that extend away from the bike to make it easier to access? Also, is there anything like that for the rear brake lever?

There is very little that is available aftermarket for this bike.

We have one member on this forum who suffered an ankle injury in a car crash, and access to the shifter was tough for them.  I took the rubber cover off, cut off the ball on the end, welded on a stainless bolt the same diameter as the stub.....the installed a longer rubber and an acorn nut on the end.

You most likely could take the brake pedal off and reshape it a bit so that is extends farther out.....or bolt or weld on an extension.

There might be a swap from another bike. Figuring that out might be hard.
Unless you're really sure about what you are gonna do to it, you might be ahead looking for a used shift lever to butcher on and see how it goes.

On another bike that I own I made an extension by removing the rubber and locating a piece of tubing or pipe and sliding that over it.  I notched it to fit around the lever which made it more secure and then I drilled a hole in it and tacked on a nut so I could run a set screw in to lock it in place.  When I sell it I'll be able to simply remove the extension and replace the rubber and it will be stock again.

I'm looking more to just extend the shifter and break pedal out away from the center of the bike more

I'm not quite sure I understand what you want.
Do you want an extension on the shift lever where your toe touches it or do you want the whole arm moved out? Or both? Do you want a bigger pad on the brake pedal, one that reaches out? Like a miniature Gas Pedal, laid over?
I hafta swing my toe out to get it out frummunda the shifter now. It's almost too long now for me.
Are you having a hard time getting your toe under it or do you ride with toes pointing out and want to be able to hit the shifter and brake without pointing feet straight ahead?
Whatever the reason for wanting to change it, it's not gonna be a Go Buy the parts
Solution. It's,
Look at it, envision the solution, build to suit your needs.
If you don't weld or braze, you can get the parts made and haul them to a welder. You'd be surprised at how reasonable they can be.
A guy tigged my header back together after I sliced it up to remove the inner pipe.
Was gonna be forty bucks, but when we got done working he just shrugged and said it's cool, and did it for free.

I extended the shift pedal once by cutting and welding a 1 inch piece in the curved section. Came out OK but wasn't chromed. Think it got sold to someone here on the board a few years ago.
 You can do a lot with a welder and some scrap metal.
